What You Will Be Doing
The purpose of the job is to provide a professional, efficient and high quality service for customers of Your Trust with long term health conditions. This includes working with the wider Development and Leisure Teams to develop, implement and improve the offer to customers, delivering as part of our Exercise Referral Scheme, undertaking health and lifestyle consultations with referral clients, informing, educating and encouraging participants on how they can make changes to their lifestyles leading to long term health improvements, ensuring knowledge of wider support available is maintained and managing confidential data.
What We Need From You
Qualified to Level 3 or equivalent in a health, wellbeing or sports related subject
Hold Level 3 GP Referral qualification
Experience of developing and delivering health, wellbeing and fitness opportunities
Experience of working with a variety of people, including those with complex health or medical requirements
Ability to use own initiative and work flexibly to deliver the service and as part of a Team
Proven ability to develop positive relationships with stakeholders, partners and members of the public
Wide understanding of health conditions and the implications for delivering safe, effective and appropriate physical activities
Understanding of how to support clients with medical conditions not yet under control
Excellent communication; both oral and written and excellent interpersonal skills and report writing
