Salary: £23,951.38 – £25,349.20 DOE (Inclusive of 14% unsocial hours enhancement)
Annual Leave Entitlement: Initial 33 days annual leave (including bank holiday) increasing to 35 days pro rata after 1 year

About the role
Our MDTA’s work within the community team to support practitioners (registered nurse, Physiotherapist, Occupational therapist, social worker, Clinical Nurse Specialist), to provide person-centered support, care and treatment to patients, their families and carers across the community who have palliative and end of life care needs.
The ideal candidate for this role will be able to demonstrate experience as a health care assistant in a clinical environment and be confident in contributing to the multi- disciplinary plan of care for patients, families, and carers for whom the post holder has delegated responsibility.
As part of the MDTA team you will assess non-complex symptoms which may include pain, shortness of breath, mobility, pressure risk, continence, psychological factors, financial issues. You will be required to analyse this information and carry out non-complex interventions within own scope of practice and refer on to specialists as needed.
The role will require travelling across West Somerset including into rural communities and working unsocial hours, evenings, weekends, and bank holidays.
